3．海外添付文書における記載状況
本薬について、海外（米国、英国、加国及び豪州）におけるそれぞれの添付文書の記載状況を
確認した 1。
これら海外の添付文書すべてにおいて、本薬は妊婦禁忌とはなっていない。潜在的有益性がリ
スクを上回る場合にのみ使用するべきと記載されている。英国、豪州及び加国の添付文書には、
一般的に妊娠中の β 遮断薬の使用は、胎盤血流量の減少による胎児死亡や早産、胎児及び新生児
の有害事象（低血糖、徐脈等）のリスクがあること、β 遮断薬使用が必要とされる場合には、β1
選択性薬剤が望ましいことが記載されている。また、胎盤血流量や胎児の発育のモニタリングを
行い、異常が認められる場合には代替治療が推奨されること、新生児を注意深く観察する必要が
あることが記載されている。

INDICATIONS AND USAGE
Bisoprolol fumarate is indicated in the management of hypertension. It may be
used alone or in combination with other antihypertensive agents.
用法・用量
DOSAGE AND ADMINISTRATION
The dose of BISOPROLOL FUMARATE must be individualized to the needs of the
patient. The usual starting dose is 5 mg once daily. In some patients, 2.5 mg
may be an appropriate starting dose (see Bronchospastic Disease in WARNINGS).
If the antihypertensive effect of 5 mg is inadequate, the dose may be increased
to 10 mg and then, if necessary, to 20 mg once daily.

Pregnancy Category C.
In rats, bisoprolol fumarate was not teratogenic at doses up to 150 mg/kg/day
which is 375 and 77 times the MRHD on the basis of body-weight and bodysurface-area, respectively. Bisoprolol fumarate was fetotoxic (increased late
resorptions) at 50 mg/kg/day and maternotoxic (decreased food intake and body
weight gain) at 150 mg/kg/day. The fetotoxicity in rats occurred at 125 times
the MRHD on a bodyweight-basis and 26 times the MRHD on the basis of bodysurface-area. The maternotoxicity occurred at 375 times the MRHD on a bodyweight basis and 77 times the MRHD on the basis of body-surfacearea. In
rabbits, bisoprolol fumarate was not teratogenic at doses up to 12.5
mg/kg/day, which is 31 and 12 times the MRHD based on body-weight and bodysurface-area,
respectively,
but
was
embryolethal
(increased
early
resorptions) at 12.5 mg/kg/day.
