In this study, we examined the construct validity, concurrent validity concerning other standard scales,
intrarater reliability, and changes in scores at 12 weeks of the previously developed ABC Dementia Scale (ABCDS), a novel assessment tool for Alzheimer's disease (AD).Data were obtained from 312 patients diagnosed with
either AD or mild cognitive impairment. The scores on the ABC-DS and standard scales were compared. The 13
items of the ABC-DS are grouped into three domains, and the domain-level scores were highly correlated with
the corresponding conventional scales. Statistically significant changes in assessment scores after 12 weeks
were observed for the total ABC-DS scores. Our results demonstrate the ABC-DS to have good validity and
reliability, and its usefulness in busy clinical settings.

We established the diagnostic accuracy of the “ABC Dementia Scale” (ABC-DS) for Alzheimer's disease (AD),
which concurrently assesses activities of daily living (“A”), behavioral and psychological symptoms of
dementia, and cognitive function, using a novel scoring approach called the three-dimensional distance (TDD).
The TDD accurately discriminated the AD stages and detected score changes indicating disease progression over
12 weeks.The ABC-DS is stable, accurately stages AD severity, and monitors disease progression. The TDD is a
useful algorithm for detecting disease progression.

We used item response category characteristics curves (IRCCC) to investigate the responsiveness of five
dimensions in five items of the EQ-5D-5L over the possible range of the QOL. We also calculated the
correlation coefficient between EQ-5D-5L and ABC-DS, and obtained a regression model to estimate the expected
utility score using the ABC-DS score. IRCCC indicated that three questions of the EQ-5D-5L for mobility, selfcare, and usual activities accurately rated the QOL; however, the other two questions for pain/discomfort and
anxiety/depression did not sufficiently contribute to the rating. EQ-5D-5L utility scores correlated well with
the three-dimensional distance (TDD) scores of the ABC-DS. By using a regression model to estimate the
expected utility score of EQ-5D-5L by TDD, our simulation estimated that the therapeutic effect obtained by
revising a drug regimen resulted in a monetary benefit of 0.002 QALY on average. We confirmed the concurrent
validity of EQ-5D-5L by proxy rating with the ABC-DS. Although two items of EQ-5D-5L did not contribute to
estimating the QOL, the other three items did, thus reflecting the severity of AD.

The ABC-DS scale identified subgroups of patients with global clinical dementia rating of 1,2, and 3.
Considering our results in conjunction with the clinicalexperiences of the AD expert among the present authors
regarding longitudinal changes in ADL, BPSD, and cognitive function, we were able to propose potential
progression pathways of AD in the form of a hypothetical roadmap.

The purpose of this study was to reevaluate the standard validity of ABC-DS separately for various dementia
types and severities. Although the ABC-DS targets AD, it can be used in VaD based on the results of this
study. In other types of dementia, the results differed depending on the domain; in some conditions, the ABCDS may not show sufficient concurrent validity with other standard scales. Also, the ABC-DS is more beneficial
for moderate-to-severe dementia.
